How to Mail In your device for repair:
1. Please download and fill out the online form below for any Mail-In Repair requests. You can fill out one form for multiple devices. Email a copy to [email protected] and print a copy to put in your shipping package. Please fill in all blanks in red and initial all disclaimers and sign (Must be 18 or older).
2. Then ship out to the address at the top left of the form.
3. Email me the tracking number using the Contact page HERE
4. Once the repair is completed, I will send a video to you showing the repair is completed, so please include an email for the video to be sent to… review the video and if happy with the repair, sign the bottom of the disclaimer (Above the repair authorization Signature on bottom … showing you are satisfied & I will accept payment through Paypal or Venmo (No Credit Cards or Checks) as these are currently my only payment methods available.
